When EPA ID Number indicates an inactive status, you may re-activate or apply for a new number by submitting a Form 1358.

1. DTSC Form 1358 (EPA ID Number Application) is available on the Internet at www.dtsc.ca.gov.

2. Select "EPA ID Numbers" from the Quick Links menu.

3. Under "Identification (ID) Numbers" select the link to "DTSC Form 1358".

IMPORTANT: Do not mail DTSC Form 1358 with your Verification Questionnaire forms. Your EPA ID Number Application will not be processed unless you mail, fax or email it SEPARATELY to the address, fax number or email address provided on page 4 of DTSC Form 1358.
